chore: relicense gortex under Apache 2.0

Replace the PolyForm Small Business 1.0.0 derivative with the standard
Apache License 2.0 text. Drops the commercial-license threshold model,
the competing-product carve-out, and the contributor commercial license
in CONTRIBUTORS.md. README license section and the Claude Code plugin's
description / link / embedded LICENSE body are realigned to match.
