    • Add shell_activation config (always/agents-only/off) for non-invasive shell hooks
    • Add .lean-ctx-id file support for explicit project identity (Docker collision fix)
    • Migrate graph_index, semantic_cache, bandit, embedding_index to composite hashes
    • Fix security hardening test for stricter token validation
    • Update CHANGELOG with all v3.5.16 additions
