    Security: SHA-pinned all GitHub Actions, fixed TOCTOU race in VS Code
    extension, eliminated stale CodeQL Python config, added email masking.

    Fixes: TypeScript .js→.ts import resolution (#146), graceful MCP client
    disconnect, unique session IDs with atomic counter.

    Improvements: env var forwarding in pi-lean-ctx (PR #144 by @glemsom).
