    Auto-detect IDE/agent during MCP initialize and switch to Unified Mode
    (5 tools instead of 24) for 33+ known clients. Saves ~16K tokens per
    session. Unknown clients safely fall back to full tools. Enhanced ctx()
    meta-tool with inline sub-tool reference. LEAN_CTX_FULL_TOOLS=1 to
    override. 4 new unit tests.
