    lean-ctx setup now injects tool-preference rules into every detected
    AI tool's global configuration, ensuring agents use lean-ctx MCP tools
    instead of falling back to native equivalents. Supports Claude Code,
    Codex CLI, Cursor, Windsurf, Gemini CLI, VS Code/Copilot, Zed, Cline,
    Roo Code, and OpenCode. Rules are append-only and idempotent.
