    • Fix MCP instructions: clearly separate REPLACE (Read/Shell/Grep) from
      KEEP (Write/StrReplace/Delete/Glob) tools. Agents no longer block on
      Write without prior ctx_read (#20)
    • Fix lean-ctx doctor on Windows: prefer .exe over .cmd shim from
      where.exe, add fallback candidates for OS Error 193
    • stats.json missing is now a yellow warning (not red error) on fresh
      installs
    • lean-ctx gain shows hint to run lean-ctx setup when MCP/CEP shows
      0% savings (shell hook only)
    • Enable GitHub Discussions (#19)
