    • lean-ctx update now refreshes shell aliases in ~/.zshrc, ~/.bashrc,
      config.fish automatically (fixes pipe guard not being applied after
      binary-only updates)
    • Multi-shell support: all detected shells are refreshed, not just one
    • Thinking budgets tuned: FixBug→Medium, Debug→Trace, Review→Trace,
      Explore→Minimal
    • README: added "Updating lean-ctx" section + pipe guard troubleshooting
    • Deploy checklist: added Shell Hook Refresh + README/GitHub sections
