    • Hook redirect now detects 50+ binary extensions (images, PDFs,
      archives, fonts, videos, compiled files) and passes them through
      to native Read. Fixes #67 where lean-ctx blocked image viewing.
    • Merge PR #66 (@DustinReynoldsPE): disabled_tools config field
      excludes unused tools from MCP tool list for token reduction.
    • Close #65: clarified shell hook vs MCP server caching.
    • Bump to v2.21.0. 501 tests passing.
