    Hook logic (rewrite + redirect) now runs natively in the lean-ctx binary
    via lean-ctx hook rewrite and lean-ctx hook redirect. Eliminates
    Windows Bash dependency that caused PreToolUse:Read hook errors.
    Existing .sh-based configs auto-migrate on next server start.
