    Windsurf + Claude Code extension doesn't have a native Read tool, but
    Edit requires it. Agents entered infinite loops burning 10%+ quota.

    ctx_edit provides search-and-replace as an MCP tool: reads, modifies,
    and writes files in one call. Instructions v7 adds anti-loop guidance.

    Bumps to v2.16.5.
