    Claude Code Desktop's Edit tool requires native Read to be called first,
    but lean-ctx's instructions forbade native Read entirely. Agents got stuck
    and fell back to full-file Write rewrites.

    All injected rules now use "PREFER lean-ctx tools" instead of "NEVER use
    native tools", with explicit Edit compatibility notes. Rules v5 → v6.

    Bumps to v2.16.4.
