- Add escape mechanism to persistent-mode.mjs (max 10 reinforcements for ultrawork, max 15 for todo continuation)
- Fix pre-tool-enforcer to check global Claude Code todos directory (~/.claude/todos/)
- Fix /cancel-ralph to actually write state file with {active: false}
- Flesh out tiered agent definitions with proper behavioral instructions:
- oracle-low, oracle-medium
- librarian-low
- sisyphus-junior-low, sisyphus-junior-high
- frontend-engineer-low, frontend-engineer-high
- explore-medium
