    v0.3.8: rebuild system prompt on /setup and /model switch

    Previously /setup and /model reused the old system prompt, so the model
    would still identify as the previous model (e.g. "I am Claude" after
    switching to GPT). Now both commands rebuild the system prompt with the
    new model identity.
