    • All path resolution: HOME → USERPROFILE fallback for Windows
    • Bash tool: cmd /C on Windows instead of sh -lc
    • fs::rename: remove target first on Windows (rename fails if exists)
    • var_os("HOME") in config/oauth/remote: added USERPROFILE fallback
    • /setup: "Skip reviewer" clears reviewer_model
    • /setup force: clears all reviewer env vars (OPENAI/GEMINI/GLM/etc.)
