Keep PEM loading and installation-token provider construction at the CLI leaf, then pass a generic refreshing token provider through the existing HTTP transports. Rebase the feature onto current main and keep the HTTP command unchanged.

Sam Morrow e05a38403f feat(auth): add GitHub App server-to-server authentication for stdio
Add non-interactive GitHub App installation authentication to the stdio
server, so headless deployments (CI, Kubernetes, background agents) can
authenticate without a browser, device code, or elicitation. This is the
outstanding follow-up tracked in #1333: OAuth login shipped the interactive
user-to-server flows, but PEM-based server-to-server auth was still needed to
remove the interactive requirement.

The new internal/githubapp package signs a short-lived RS256 JWT with the
app's private key, exchanges it for an installation access token, and refreshes
it transparently before expiry. It exposes a Provider whose AccessToken method
mirrors oauth.Manager so it plugs into the existing BearerAuthTransport token
provider. Only the standard library and golang.org/x/oauth2 are used.

The private key is injected safely: a file path (GITHUB_APP_PRIVATE_KEY_PATH,
preferred — mountable as a secret and kept off argv and out of the environment)
or an inline GITHUB_APP_PRIVATE_KEY env var. There is intentionally no flag for
the key contents, which would otherwise leak via the process command line.

App auth is mutually exclusive with a PAT and with OAuth login. A loud startup
warning and a dedicated docs page (docs/github-app-auth.md, with Docker and
Kubernetes examples) cover the security considerations: this injects a
high-privilege credential alongside the agent and is not recommended without an
independent security review.

* Add opt-in intent-aware Copilot issue assignment tool

Add `assign_copilot_to_issue_with_intent` in a new non-default toolset
`copilot_issue_intents`. The tool uses GraphQL's object-form
`assignees: [AssigneeUpdateInput!]` so intent metadata (rationale,
confidence, is_suggestion) is attached to the Copilot entry only, while
existing assignees are preserved.

- Reuses the existing Copilot actor lookup, target-repository
  resolution, base_ref, custom_instructions, GraphQL-Features header,
  and direct-assignment PR polling behavior.
- `is_suggestion: true` records a pending Copilot assignment intent,
  returns a suggestion-shaped result, and does not launch Copilot or
  poll for a linked PR.
- `rationale` is capped at 280 characters (schema + runtime);
  `confidence` is validated against `LOW`/`MEDIUM`/`HIGH`.
- Toolset is non-default so its inputs do not add schema bloat to the
  default tool surface; available via `copilot_issue_intents`,
  `all`, or explicit tool selection.

Includes unit tests for direct assignment (with existing assignees and
with base_ref/custom_instructions), the suggestion path, invalid
rationale length, invalid confidence, and Copilot-not-available; a
generated toolsnap; regenerated docs; and an e2e test for the
suggestion path.

Per review feedback on #2909:

- Trim the tool description to mirror assign_copilot_to_issue and add
  "Prefer this tool over assign_copilot_to_issue when available",
  removing the verbose is_suggestion narrative from the schema.
- Make rationale, confidence, and is_suggestion required inputs (schema
  and runtime). is_suggestion is now always sent explicitly on the
  Copilot AssigneeUpdateInput entry.
- Update unit tests to supply the newly-required fields and cover the
  missing-rationale and missing-confidence rejection paths.

* Address review: dedupe copilot, require is_suggestion, update doc link

Per @RossTarrant review feedback on #2909:

- Reject requests where `is_suggestion` is omitted from the raw args
  before decoding. `mapstructure.WeakDecode` defaults missing bools
  to false, which would silently launch Copilot instead of recording a
  suggestion. Presence-check the raw map so callers make the choice
  explicit.
- Skip the copilot-swe-agent actor when copying existing assignees so
  we don't send its actorId twice (once without metadata and once with
  intent metadata) when Copilot is already assigned.
- Update the stale about-assigning-tasks-to-copilot reference to the
  redirect target (about-cloud-agent). Applied to the const message,
  both tool descriptions, and the e2e/unit-test literals that assert
  on that message.

New unit tests cover the missing-is_suggestion rejection and the
copilot-dedup behavior. Regenerated toolsnaps.

* Enrich issue_read get with hierarchy relationship signals

The default issue_read `get` payload surfaced no hierarchy data, forcing
agents to drop to raw REST (parent_issue_url) or scan sibling sub_issues
to discover relationships. Enrich `get` with a layered, zero-extra-round-trip
relationship signal derived from a single combined GraphQL query:

- has_parent / has_children: cheap, always-emitted routing booleans
  (addresses Sam Morrow's #2726 review note).
- parent: compact ref (number/title/state/url/repository) mirroring the
  existing get_parent payload keys; omitted when there is no parent.
- sub_issues_summary: native subIssuesSummary counts (total/completed/
  percent_completed); omitted when there are no sub-issues.

The single-issue field-values GraphQL call in GetIssue is replaced by one
combined query (fetchIssueReadEnrichment) returning field values + parent +
subIssuesSummary, so `get` adds no round-trips. Enrichment is best-effort:
a query failure still returns the base issue and never fails `get`. Parent
titles are sanitized (parent may be cross-repo) and redacted under lockdown
mode unless the parent content can be verified as safe; numeric/structural
fields and counts stay intact. get_parent / sub_issue_write behavior is
unchanged; tool descriptions clarify hierarchy is read here but written via
sub_issue_write (no writable parent field).

lockdown patterns: rather than introducing a third, redaction-with-sentinel
behavior, omit the whole parent reference when its (possibly cross-repo)
content cannot be verified safe. This mirrors how unsafe comments, sub-issues,
and PR reviews are filtered out. has_parent stays true so an agent can still
route to get_parent.

Implement three granular-only tools for adding emoji reactions:
- add_issue_reaction: Add reaction to an issue
- add_issue_comment_reaction: Add reaction to an issue comment
- add_pull_request_review_comment_reaction: Add reaction to a PR review comment

All tools are feature-flagged with FeatureFlagEnable set to enable them
only when clients request granular toolsets. Tools use go-github's
Reactions service and return minimal ID response on success (HTTP 201).

Surface OAuth 2.1 login as a first-class option above the Personal Access

The github.com OAuth example needs no token; in Docker it publishes a fixed
callback port to loopback (-p 127.0.0.1:8085:8085). The PAT block is retained
below it and noted as taking precedence over OAuth.

Sam Morrow 8ce77e3c16 feat(oauth): add stdio OAuth 2.1 login core library (1/4) (#2704)
* feat(oauth): add stdio OAuth 2.1 stdio login
Introduce internal/oauth, a self-contained library that performs the
user-facing GitHub OAuth login the stdio server uses to obtain a token
without a pre-provisioned PAT. It is independent of MCP: client concerns
(elicitation) sit behind the Prompter interface so the flows are testable
without a live session.

What it provides:
- Authorization-code + PKCE flow with a local loopback callback server,
  state/CSRF validation, and XSS-safe result pages.
- Device-authorization flow as a fallback (headless, containers).
- A Manager that selects the most secure available channel
  (browser auto-open -> URL elicitation -> last-resort user action),
  runs a single flow at a time, and exposes a refreshing token source.

Both GitHub OAuth Apps and GitHub Apps are supported without special
casing: the token is modeled as an x/oauth2 refreshing TokenSource, so
expiring GitHub App user tokens are renewed transparently (the gap that
made a stored-token approach silently die after ~8h).

When a client lacks secure URL elicitation and the flow falls back to a
tool-response message, the message advises the user that their agent/CLI/
IDE does not appear to support URL elicitation and suggests requesting it
for improved security.

Tests exercise real protocol behavior against an httptest GitHub stand-in:
PKCE challenge/verifier, GitHub App refresh-on-expiry, device polling,
URL elicitation, declined prompts, the last-resort action with advisory,
and single-flight concurrency.

- createGitHubClients uses BearerAuthTransport (and skips go-github's
  WithAuthToken, which would pin a static token) when a TokenProvider is set.
- RunStdioServer starts without a token and installs receiving middleware
  that runs the authorization flow on the first tool call, surfacing the
  auth URL or device code via elicitation (or a tool result as a fallback).
- Tool filtering uses the requested OAuth scopes; the default supported set
  hides nothing, while a narrower --oauth-scopes both narrows the grant and
  filters tools accordingly.
- A sessionPrompter adapts the MCP server session to oauth.Prompter, keeping
  the authorization URL off the model's context.
- New stdio flags: --oauth-client-id/-client-secret/-scopes/-callback-port.

This is stdio-only and deliberately does not touch MCP-HTTP auth.

* Distinguish undeliverable auth prompts from user declines

An elicitation prompt that the client cannot deliver (a transport or
protocol failure) was treated the same as a user actively declining: any
display error cancelled the flow. That conflated a system failure with a
deliberate "no", so a client that advertised URL elicitation but failed
to deliver it would hard-fail the login instead of degrading.

Add an ErrPromptUnavailable sentinel alongside ErrPromptDeclined and have
the MCP adapter return it when Elicit fails at the transport level. The
manager now falls back to the manual user-action channel on an
undeliverable prompt (keeping the background flow alive so the user can
still authorize out of band), while a genuine decline still aborts. A
context-cancelled prompt is checked first so an ending flow is never
misread as a transport failure.

Bryan Zwicker 9430064a2b Add get_parent method to issue_read (#2726)
Add an upward parent read to issue_read, the counterpart to the existing
downward get_sub_issues. Uses the GraphQL Issue.parent field and returns a
null parent when the issue is not a sub-issue. Kept always-on (not feature
gated) to mirror get_sub_issues; the dependency tools remain flag-gated.

Previously, omitting the `private` parameter on create_repository
defaulted the new repository to public, an insecure default that could
unintentionally expose code, configuration, and history. Omission now
defaults to a private repository; public repositories are only created
when `private` is explicitly set to false.

Timur 7e79ae9aa5 feat: replace include_diff with detail enum on get_commit
Replace the get_commit tool's two boolean flags (include_diff,
include_patch) with a single detail enum: none / stats / full_patch.

Why:
- The two-boolean shape had an awkward dependency
  ("include_patch only applies when include_diff is true") and an
  impossible state (include_patch=true, include_diff=false) that was
  silently ignored.
- A single discriminator collapses three meaningful response shapes
  into one orthogonal choice, makes the most expensive option
  ("full_patch") self-describing, and eliminates the "diff vs patch"
  naming confusion.

Behavior:
- Default ("stats") matches the previous default
  (include_diff=true, include_patch=false): per-file metadata with no
  patch text. Existing callers using defaults are unaffected.
- "none" omits Stats and Files entirely (was include_diff=false).
- "full_patch" is the new opt-in level that adds the unified diff to
  each MinimalCommitFile.

Breaking change: callers that previously passed include_diff or
include_patch must switch to detail. Callers using the defaults are
unaffected.

Changes:
- Added Patch field to MinimalCommitFile.
- Added commitDetail type, parseCommitDetail, and migrated
  convertToMinimalCommit to take a commitDetail.
- Updated get_commit schema, list_commits caller (commitDetailNone),
  unit tests, toolsnap, and README.

- install-opencode.md: covers OpenCode's 'mcp' config block (type-discriminated local/remote, command-as-array, 'environment' instead of 'env'), the 'oauth: false' opt-out needed when using a PAT, the '{env:VAR}' interpolation pattern, and the per-agent tool-gating pattern recommended for token-heavy servers like GitHub.

Sam Morrow 014fd17fa3 feat: gate issue_write and get_issue behind remote_mcp_issue_fields flag
Ports the gating from PR #2553 onto main (the original merge landed on a
stack base that did not make it to main).

Changes:
- pkg/inventory: FeatureFlagDisable becomes []string (any-listed-on → hide).
  FeatureFlagEnable stays as a single string. This avoids the AND-of-enable
  semantics from the earlier proposal, which encoded dependencies rather
  than rollout knobs and had no real call site. Disable-OR is the case
  that does need the slice (LegacyIssueWrite below).
- pkg/github/issues.go: split IssueWrite into IssueWrite (flag-enabled,
  exposes issue_fields) and LegacyIssueWrite (flag-disabled, omits it).
  Both register as 'issue_write'; mutually exclusive flag annotations
  pick exactly one at runtime. Refactored into a shared buildIssueWrite
  helper instead of duplicating the ~250-line tool definition.
- pkg/github/issues.go: GetIssue field_values enrichment now requires
  the flag at runtime. The verbose REST IssueFieldValues is always
  cleared from the response.
- Existing single-flag Disable call sites converted to slices.
- New toolsnap variant issue_write_ff_remote_mcp_issue_fields.snap; the
  canonical issue_write.snap is owned by LegacyIssueWrite.
- README + flag docs regenerated.

- Extract newMinimalCommitFromCore to share field mapping between
  convertToMinimalCommit (RepositoryCommit) and the new
  convertCommitResultToMinimalCommit (CommitResult), removing ~50
  lines of duplicated logic from the search_commits handler.
- Add MinimalRepoRef and a search-only MinimalCommitSearchItem type
  (embedding MinimalCommit) so cross-repo commit search results
  identify the repo each commit came from. Keeping the field off
  MinimalCommit avoids paying for a never-populated field on the
  get_commit/list_commits output types.
- Rewrite the query description to teach the model the actual
  commit-search qualifier surface (repo:/org:/user: scoping, author/
  committer/date qualifiers, hash/tree/parent, merge:, is:public)
  and reword the sort description to drop redundancy with the enum.
- Extend tests to assert the repository field is surfaced and to
  cover commits with no resolved GitHub user (nil Author/Committer).
- Refresh README and toolsnap.

Sam Morrow e9533330b0 fix(search_code): tighten query description for accurate model guidance (#2513)
The current `search_code` query description is hand-wavy and gives the
model little usable guidance on GitHub code search syntax, which (per
analysis in #2390 across thousands of agent sessions) leads to repeated
422 ERROR_TYPE_QUERY_PARSING_FATAL responses from agents that guess at
plausible-but-invalid syntax.

Re-applies the spirit of #2442 by @jluocsa, originally suggested by
@danmoseley in #2390, but corrected against the actual endpoint this
tool calls.

Critically, this tool uses go-github's `client.Search.Code`, which hits
the legacy REST `/search/code` endpoint — NOT the new code search
("Blackbird"). Verified against the live API:

  symbol:WithContext repo:github/github-mcp-server  -> 0
  /Get|Set/ repo:github/github-mcp-server           -> 0
  path:**/*.go func repo:github/github-mcp-server   -> 0
  filename:*.md repo:github/github-mcp-server       -> 0
  (Foo OR Bar) -path:vendor language:go             -> 422

So `symbol:`, `/regex/`, path globs, filename globs, and parenthesized
boolean groups — features the proposal in #2442 listed — silently
return zero or fail. Documenting them would teach the model syntax
that doesn't work on this endpoint.

The new description focuses on what's actually supported by legacy
code search and the real bugs observed in #2390:

- `path:dir` is a prefix, NOT a glob (displaces `path:**/*.ts` guesses).
- `filename:exact.ext` is exact, NOT a glob (displaces `filename:*.md`).
- `/regex/` and `\|` inside quotes don't work — call this out so the
  model stops generating them.
- `symbol:` doesn't work on this endpoint — call this out.
- Parenthesized boolean groups 422 — call this out so the model
  stops wrapping `OR` chains in parens.
- Adds `extension:`, `in:file`, `in:path`, `size:`, `filename:`, `user:`
  qualifiers that the previous text omitted.
- Implicit AND, `OR`, `NOT`, and `"quoted phrase"` for exact match are
  documented positively.
- 256-char query limit.

All four examples in the new description are verified against the live
GitHub API and return non-zero results.

Sam Morrow 0f0506d2fd refactor: remove dynamic toolsets and deprecated closure constructor (#2512)
Dynamic toolset discovery (the meta-tools enable_toolset, list_available_toolsets,
get_toolset_tools and the --dynamic-toolsets / GITHUB_DYNAMIC_TOOLSETS switch)
was a local-only feature never offered by the remote server. Removing it
deletes a meaningful chunk of branching, configuration surface and tests
for a path no longer in active use.

The deprecated closure-based NewServerToolWithDeps generic constructor was
only kept around for the dynamic tool registration path and is removed
together with it. Going forward there are exactly two constructors:

- NewServerTool — raw mcp.ToolHandler, no closure, no unmarshalling
- NewServerToolWithContextHandler[In, Out] — typed handler, deps via context

Inventory methods that only existed for the dynamic path
(ToolsForToolset, IsToolsetEnabled, EnableToolset, EnabledToolsetIDs)
are removed. ResolvedEnabledToolsets loses its dynamic flag.

Also strips dynamic references from the README, server configuration
docs, copilot-instructions, mcp-diff workflow, and conformance-test
script.

John CSA bafcaf57c3 fix(pull_request_read): expose after cursor parameter in input schema (#2489)
The `pull_request_read` tool description tells clients that
`get_review_comments` uses cursor-based pagination (`perPage`, `after`),
and the handler does plumb `after` through to the GraphQL query, but the
input schema only declared `page` and `perPage` (via `WithPagination`).
Because `after` was not advertised in `inputSchema`, MCP clients had
no way to request it, leaving cursor pagination effectively broken:
`perPage: 1` returned only the first thread with no way to advance, and
`page` was silently ignored by the GraphQL path.

This change adds `after` to the schema (string, optional) with a
description making clear it only applies to `get_review_comments`. All
other methods continue to ignore it. No handler behavior is changed.

- Add `after` schema property after `WithPagination` in `PullRequestRead`
- Regenerate `__toolsnaps__/pull_request_read.snap` and update README
- Add a regression test asserting `after` is in the schema and a new
  table-driven case verifying the cursor is forwarded to the GraphQL query

Fixes #2122 (for the `get_review_comments` pagination part). The
remaining concerns in #2122 about unbounded response sizes for `get`,
`get_diff`, and `get_reviews` are deferred to follow-up design.

Adds `resolve_thread` and `unresolve_thread` methods to the
`pull_request_review_write` tool, enabling users to resolve and
unresolve PR review threads via GraphQL mutations.

- Add ThreadID field to PullRequestReviewWriteParams struct
- Add threadId parameter and new methods to tool schema
- Implement ResolveReviewThread function using GraphQL mutations
- Add switch cases for resolve_thread and unresolve_thread methods
- Add unit tests covering success, error, empty and omitted threadId
- Document that owner/repo/pullNumber are unused for these methods
- Document idempotency (resolving already-resolved is a no-op)
- Update toolsnaps and generated docs
