Trevin Chow 12f692ec1f fix(skills): make model-invoked descriptions context pointers ...

The size-restructure sweep left most descriptions untouched, so catalogs,
flags, and procedure still sat in the always-on skill list. Restate the
15 failing descriptions as identity plus one trigger per distinct branch,
and move the ce-work discovery pin off the caller-flag string.
